Getting Started

You'll need to tell us:

  • who your complaint is about
  • what you think has been done wrong
  • basic personal details

You'll need to attach any relevant documents as evidence to support your case.

There are ten steps to complete the form. You will be unable to save the form if it is incomplete.

It takes about 15 minutes, on average, for people to fill in and submit this form. For some people, it may take longer due to internet connections, how much detail you enter, etc.
